California birth certificate letter question

I was born in California and want to fix my birth certificate now.  I have a name change order from my friendly neighborhood circuit court and now need a physicians letter.

So the question is: does it have to be signed by an MD?  Or can it be done by a nurse practitioner?  Does anyone know for sure?
