Urgent question for help

Dear all,

I have scheduled my SRS with Dr. Chettawut in Bangkok Thailand in this December. Now I have started to prepare all the necessary legal forms. Today, I received the following message from the doctor's office, in addressing my question on the SRS certificate they will issue to me.


"We provide the certificate (1 laminated and 1 A4 paper) in English.
....
Even if the certificate is in English, some states requires the notarized or certified SRS certificate so you need to check the rules and regulations first to be sure."

I live in Honolulu, Hawaii. At this point, I have not checked with the state on this regulation. So I'm wondering do we have to notarize the SRS certificate? Or the original certificate is sufficient. I know a lot of sisters on this forum had the similar experience before. And I really appreciate any answer to this important question.

I live in Florida. The surgeons letter does not need to be notarized. It must have the surgeons letterhead, licence number and address and a few other items.

Each state should have a website with the requirements listed.

My doctor gave me a packet of letters for the different agencies, I know the birth certificate letter is notarized, I don't think the Social Security and DMV letters were, though.

Thank you so much Dani and Devlyn! Really appreciate it and feel like living in a big family!

I think I will notarize every required letters/forms just for safety. Much easier to do it in Bangkok.

Were you born in Hawaii?  If so, surgery is not a pre-requisite to change the gender on your birth certificate, as you can see from the following link:
