Dear all,
I have scheduled my SRS with Dr. Chettawut in Bangkok Thailand in this December. Now I have started to prepare all the necessary legal forms. Today, I received the following message from the doctor's office, in addressing my question on the SRS certificate they will issue to me.
"We provide the certificate (1 laminated and 1 A4 paper) in English.
....
Even if the certificate is in English, some states requires the notarized or certified SRS certificate so you need to check the rules and regulations first to be sure."
I live in Honolulu, Hawaii. At this point, I have not checked with the state on this regulation. So I'm wondering do we have to notarize the SRS certificate? Or the original certificate is sufficient. I know a lot of sisters on this forum had the similar experience before. And I really appreciate any answer to this important question.
