Narrowed my search to two surgeons for in-person evaluations.  One in Boston, one in Spain.  I also considered Argentina.  The list I came up with might not be the right answer for others as we each have different situations, goals, genetics.

Seattle to London to Málaga.  Arrived after sleeping Seattle to London coach on Virgin, then Sleepless on EasyJet.
I'm glad EasyJet doesn't offer surgery... they follow safety regulations but anesthesia might be interrupted every 45 minutes with advertising announcements, as they did on my flight.

To avoid risk of a late flight I arrived the night before, stayed near Málaga airport - headed into town on the subway.  FacialTeam arranged a car & driver to meet 9am in the hotel lobby.  On the map it looked like a 30 minute drive but took nearly an hour - traffic wasn't stopped but a bit slow along a beautiful coastline.

Marbella's HC Hospital looks a bit like a spa, very nice area a few blocks from the ocean.  FacialTeam is one of three operations there - the other two are fertility and oncology.

The meeting is to evaluate exactly where things are right now, so I didn't wear any makeup this morning.

10am met with Sonja, patient coordinator.  Very polite and efficient.  One thing I noticed and like is she's assigned to stay as primary contact all the way through recovery, instead of being bounced among several contacts.

Filled out forms on their tablet and a very short paper form.  HC Hospital has a fast free internet connection.  Sonja took photos with a two cameras in the FacialTeam office.  One was a gizmo with a pair of laser dots to align distance, directly connected to a PC - I think that one measures 3D/surface contours.

Then back across the lawn for cat scans at HC's facility.  Remove earrings for that.

Prep work was about 45 minutes, then a break before 11am with Dr. Simon.

I had Alexandre at VirtualFFS do a full set of photos earlier this year, I brought those to the meeting.  I heard FacialTeam provides a set of VFFS simulations with an in-person eval but I felt it would be useful to bring what I already have.

Dr. Simon went over the CT scans and checked the shape of soft tissue on my face with his hands.  I didn't realize until later he was carefully observing how I speak, facial expressions - based on that he said I might not need a lip lift - he is leaning against it.  Those are things you can't gain from a static photograph.  A Skype call is not 3D, he can't reach through a Skype call to check skin elasticity or run a CT scan.  He also suggested I avoid a tracheal shave unless that's bothering me (it isn't).

He spent quite a bit of time explaining potential areas for FFS.  He placed a clear plastic page over the CT scans and drew where he would make changes - I learned quite a bit, for example testosterone during growth phase changes some but not all sections of the skull.  He showed me the natural seams that are literal dividing lines.  He looked carefully through the VFFS photos from Alexandra (on my iPad in the photo below) and used those to help explain some options.  Yep those are scans of my skull on the table.

I was previously aware of FacialTeam's unique hair incision/grafting method that seems like an excellent idea - don't waste follicles from the main FFS incision.  I suppose a potential downside is a less radical change in eyebrow position since the tension/incision is further back, but that's what I prefer.  I wasn't aware of their jaw-work methods which sound quite good.  He uses an ultrasound cutting tool instead of a saw to minimize impact on surrounding tissue - apparently that became available 5 years ago.  For the incision inside the mouth, instead of a long arc he makes three shorter cuts to improve healing time.

I definitely want a nose job and he brought in Dr. Bellinga - he focuses on that area.  They went over the different incision methods.  I had assumed any nose job requires a cut across the lower nose line but that's only if combined with a lip lift.  They prefer to work on the nose with an incision between each nostril, a very small incision and virtually invisible once it heals.

I was surprised the meeting with the surgeons was more than one hour.  I never felt rushed and didn't feel like any part of this was a sales pitch.

I made my decision, I'm going with FacialTeam for FFS.  Current waiting list is a half year - I'll be returning June or July 2018.

After the meeting they arranged a taxi at their expense to run back to Málaga - did my makeup in the car.  I took a train from Málaga to Seville.  The center part of Seville is amazing - definitely recommend if you're in the area.

Thanks, Kendra!

FacialTeam is high on my list if Kaiser FFS falls through. (Issues there that I'll post about once I get a response from them...)
Earth my body, water my blood, air my breath and fire my spirit.

My personal transition path included medical changes.  The path others take may require no medical intervention, or different care.  We each find our own path. I provide these dates for the curious.

I think you made the right decision based on your two options. And what a thorough research process you went through!

I consulted with several FFS specialists, and decided on Facial Team.  That was 3 years ago.  I'd recommend FT to anyone who needs/wants facial feminization.  Top notch hospital, talented surgical team, and excellent patient coordination and after care.  Oh, and I love southern Spain!

I liked Marbella, the Hospital and the people at Facialteam. I have only two things I somehow am considering a bit on the negative side. They tend to be too subtle - tell people they dont need this or that, usually regarding jaw or nose or take away not enough bone to make the face strongly feminine - they are usually happy reaching female ranges. i know of several patients who went back again to get more bone sheaved off or get the procedures done that they skipped the first time based on their recommendations. So I guess this menas get clear what you want by yourself and if you want to have more visible changes, tell them to be more agressive than usual. The other thing I miss with them a bit is that they do no or very few cosmetic procedures usually - they do lip lifts but no lower face/neck lifts apparently and they dont seem to recommend eyelid surgery or lip augmentation , usually. I read today that for some of these things, they dont do these themselves but call in a surgeon they know? But the things I read were inconclusive. Other than that I found them very professional, they take a lot of time for the consultations and I liked them using the CT scan to show where there is a lot of bone to be changed. Seeing my skull like this made me a bit dysphoric because it looked more typical masculine than my actual face does, with all the laers or hormonally shaped soft tissue on it :D

Hi Kendra,

Awesome description of FT's consultation. Good luck in your future FFS. Spain looks amazing.

Could you share the procedure and cost that they and Dr. Spiegel quoted?

Thanks,

Costs depend on specific procedures and each of us has different requirements... for example I don't need cheek implants - surgeons here and in Boston said my tracheal bump is quite mild and best to leave as-is.

I'm comfortable sending my estimate to any of us by PM but I won't post it for unregistered viewers.  Rachel - I'll send you the costs I was quoted.  If anyone else is curious, PM welcome.   :-)

You are quite the adventurous traveler... I tend to have to go from point A to B and want to get home ASAP... :)  Re the advertisement announcements... never heard of that happening before... LoL (on hindsight).

Hey Kendra, congrats, so happy for you!

Just out of curiosity, was a face to face consultation something that facial team requires? They were initially on my list before choosing someone else.

Also, your adventure looked incredibly fun!

I had originally scheduled a Skype consultation with FacialTeam.  And then I thought about it and realized... this is a pretty important decision.  So I went there.

FacialTeam travels to a few major international cities once per year, but in-person in Marbella is a more comprehensive consultation including CT scans.  And it's a nice place to visit.

No personal consultation is required with any surgeon I know of, so far. Except of course on the day or two days before surgery to plan it all out. BEfore that, one can usually choose - sending emails only, better have Skype maybe plus sending pictures and even better have personal consultations off-site and ideally personal consultations on-site. But since the personal copnsultations cost a lot of money for travelling, I think it only makes sense to do them with the top 2 or 3 on the list. Many dont get personal consultations at all and just rely on trust and good reputation and do it all remotely and then meet the surgeon the day before surgery to plan it all out.

Thank you Kendra for this information. It will help people like me who is contemplating doing FFS in the future.

I'm back in Marbella now for surgery tomorrow morning.  The cheapest flights from Seattle landed in London so I spent a few days driving around South Wales, and then a weekend in London before heading south.

Before returning to London, went to the Midlands for an awesome dinner at Davina's place.  She worked all day on this meal.  Before dinner, stopped and saw several cars Davina has built from scratch or restored.

Davina and one of her creations.  She built and has raced this car.

Davina (England), Sara (South Wales), Kendra (Seattle)

Quite a bit of traffic near Windsor before dumping off the rental car, something about a royal wedding.  In London, met up with Judith Lynn for a nice dinner Friday, and all of Saturday - a most enjoyable time.

...and then arrived in Marbella Spain.  Megan timed a trip to be there at the same time - we had a great day wandering around the waterfront, and of course had to have tapas for lunch.  I took advantage of her rental car and picked up extra groceries for my next couple weeks at the Aldea Health Resort / surgery recovery facility.

Puerto Banús - 2km walk from FacialTeam offices

Time for tests and measurements before the pre-surgery meeting.  A blood test, an injection to reduce the potential for blod clots during surgery, and an EKG.  Next was an updated set of facial photos taken using same picture angles and equipment as the evaluation meeting, including a visual 3D grid/measurement set using a special whizbang camera.  The previous CAT scans weren't updated since skulls don't change very fast unless you cut into them.  This entire process went pretty quick - less than a half hour total.  I had a two hour break between that and the pre-surgery meeting so I went for a walk along the ocean - 10 minute walk from FacialTeam.

The meeting with Dr. Simon and Dr. Bellinga took 45 minutes.  Dr. Simon will be doing the primary FFS, and Dr. Bellinga is a nose specialist.  They carefully checked updated measurements and went over surgery details for tomorrow - both asked several times if I had any questions or requests.  I know this will involve some pain and hassles, and I'll look ridiculous for a few days or weeks but it's all part of the process.  I am super confident in the outcome, and excited.

Procedures I am having tomorrow: Forehead skull shaping (Type 3), jaw bone trimming, nose job, lip lift, and hair transplant to fill-in two corners for a more feminine hairline.

Pre-surgery meeting.  Details, details.

This almost looks easy.  Almost.  Well, maybe not.
