I don't think there is a consistent answer and different doctors do it differently.  My doctor prescribed both for me from the beginning.

I sincerely hope that you are receiving Doctor supervised HRT.  Certainly this is a question to ask your doctor if you need an answer that is specifically for you.   In my personal case, my Doctor started me on both at the same time and adjusted the dosages after looking at the frequent blood tests he was giving me.   HRT affects every body differently and my experience was just that... "my experience" and may not apply to anyone else.
As many have stated on here regarding HRT....  "Your mileage will vary" meaning that you will have a specific regimen of hormones and drugs that are specifically for you based on blood tests and your Doctor's direction.... and your HRT results may or may not be what others are telling you that their HRT results are.

I agree with Aspiringperson that this is a question that your doctor should have an answer based of your bodies needs.
I myself was on a medication already that was the T blocker prescribed when I started hrt and E.
