Dilations after surgery

Hi, I m post op 4 month. I ve a suspect of entero vaginal fistula maybe caused by strong dilations. My problem started with a anal fissure, and then after any days I found lot of pus under my anus and fecal matter in vagina. My surgeon was Suporn and he said that this fistula can fix alone, without anymore surgeries. Here, where I am, in Italy, I did the barium enema that didn't find anything close to a fistula, but I feel a knife pain and when it happen an area inside my anus need to be discharged from an amount of pus. In the same moment when I clean the area, I find any fecal matter in vagina and copiously yellowish discharge. I don't know how can I deal with dilation regimen because when I insert the normal (32mm width) at the 16th cm depth I feel something like a wound pain. My doctors here suggest to stop dilating for 3/4 days and give a rest to the vaginal tissue, because they suppose that I stretch anything inside. My pain, I understand, is not normal but from the Suporn Clinic they don't give me a good answer for dilations. First, they told me to stop, and one email after, they told me to continue at the depth provided by doctor Suporn. I really don't know whats the best choice. I'm still waiting for the magnetic resonance and the visit  by my proctologist tomorrow 3~6~2018. Really scared about it.

I can't answer your question, but Suporn has had a couple of cases of fistula before and repaired them. Its in that pdf a former patient wrote.

Hello marydilor1,

Sorry to hear about your recovery issues, that sounds painful.  I'm glad to know you are visiting a local doctor tomorrow and it's good you also immediately contacted Dr. Suporn.  I think seeing a medical specialist soon as possible is the best thing you can do.

One thing I'll suggest is consider dilators serve two purposes - depth and width.  Depth is extremely difficult or almost impossible to recover if dilation isn't followed in the early days of recovery.  My understanding is width can be developed or potentially increased later on.  I am basing this on instructions I was given when I had GCS 6 weeks ago.  The dilator size you mentioned (32mm, 1-1/4") is a "Size 2 blue" from Soul Source Dilators - they are a common supplier.  If a "Size 1 purple" (28mm, 1-1/8") dilator or equivalent is available, temporarily using a narrower size might reduce the pain or chance of further injury while maintaining depth.  I need to emphasize this is what I'd do if I faced this situation but I cannot give any medical advice... you should follow advice from your doctors. 

Also, make sure you're using a medical grade lubricant for the first few months, and using enough of it.  With surgery I was provided a tube of McKesson Lubricating Jelly, and I bought a case of 24 tubes - the part number is MCK19191406

Thank you Kendra. I m still doing specific exam and treat my problem with some specific antibiotics (ciprofloxacin/metronidazole), wash after toilet with betadine the both areas (anus and vagina) push a little ball of cotton full of betadine under anus where is located the abscess and apply a cream specific for anal fissures and other issues. Then dilate and then use the vaginal douche with a mixture of betadine and water. Now I don't feel specific symptoms, even after dilations but I keep a full comfortable session without the L dilator (34 mm). This cause lot of discomfort after dilation, and I suppose is the cause of my trouble. With this new routine I'm quite better, no pain, not lot of discharge from vagina. I'm in contact with lot of doctors, also the SRS centre of my country who want to analyze the RMI result with medium contrast intravenous. This story is strange because doctors don't find any important issue and a possible surgery  reparation. I can maintain a 16 full depth without any pain, just one cm low than the full depth provided by Suporn. If I try to push hard I feel myself in a lot of discomfort for all day, and the discharge increase. Thank you.
