Name and Gender Marker Change via US Citizenship

To anyone who can help me...

I'm mtf transgender and I was born in Philippines. I'm 32 and have lived in the US for the last 18 years. I have my green card  renewed as a male that expires in the year 2020 and I can now finally start applying for my US citizenship, all the while I had started my transition when I was a teen  . Here's my question/request or any advice and knowledge: Do I go ahead and apply for my citizenship as female and change my name within my application process? (all with the help, of course, of a doctors note) also my Dr Marvin beltzer gave me note that says I'm changing my name  and gender.. And went through a therapist...but its says valid through 2006.? I wonder if I can still use this.? ..I have not had any bottom surgery as I don't intend to..but now I live in north Carolina...I was debating whether to see a new MD that specialized, in,transgender who can give me a new form?..I really need anyone help ..pls..I even called an attorney here in Charlotte.. Who helps lgbtq community. But she doesn't seem to be familiar with transgender immigrant like me...

If anyone has had any experience with this in the past or knows about it please let me know, any advice or answers would help.

I can't speak directly to your request but I wanted to send this link to a TED talk I watched yesterday. You may have already watched it but in it she describes changing the status of what you have mentioned because of her green card status. You may find some helpful tidbits in there, who knows.
