3 Days Post Op With Dr. Dugi at OHSU

Like the title says, I had sex reassignment surgery done with Dr. Daniel Dugi on Monday the 8th. Right now it's 2 AM and I am writing this because I can't sleep. I had minimal depth Vaginoplasty  during a surgery that took almost 8 hours.  My overall experience has been good, though I have yet to see my results as the bandages won't come off until today. My overall experience has been positive, with a few minor exceptions, and I definitely feel like I'm being well taken care of. I will post much more extensive updates once I get home, which should be later on today. Fortunately I live in the area so it's not but a five minute car ride.

Oregon Health and Science University.  I just got my catheter out, and once they remove the drains I should be able to go home. The stay has been OK, but I'm definitely looking forward to being in my own bed.

Congrats, Autumn!  I've read some of your past posts, it certainly is a major advantage to be able to have this done right here in town.  I live out in Yamhill County and I'm waiting to hear from the OHSU board.  It's taking a while, for some reason.  I've an appointment with my therapist in an hour and a half, will kid her about how her referral letter made me sound nuts.   :o ;D

Might get to meet Dugi tomorrow at the Q Center when Meltzer and Ley do their presentation, I hear he's a real southern gentleman.

Made it home okay. Really swollen and pretty uncomfortable but not too bad. The appearance is pretty good for how new it is. I will post more later, but right now I am very pleased.

As promised, here is a longer account of my surgery with Dr. Dugi. I went in on Monday, October 8th at 7 AM. Got my IV started and my partners kissed and was then wheeled back to surgery. I was out for almost 8 hours; he really takes his time with surgery, which I think shows in how well done my incisions are and how tiny the stitches appear to be. After I woke up in post-op I was really, really out of it and have little memory except for a really painful moment when a nurse mistook one of my drains (leading under the skin of my abdomen and held in place by a stitch) got a monitor or something and attempted to YANK it out. This was extremely painful and caused me to scream and bleed a bit, but apparently it didn't do any lasting harm and when the doctor examined it later he said it appeared to still be in place. This was one of the only two things I was unhappy with during my visit.  Anyway, after this I was sent to my private room and my husband and other male partner came to visit. I was offered dinner and ordered some soft food, but I was so medicated I was unable to eat and had no appetite anyway. My partner did manage to feed me a few spoonfuls of mashed potato, but I was sick soon after so I stopped trying. I fell asleep every other minute or so and wasn't really capable of much interaction.

When I woke up the next day I was much more myself, though I continued to doze off regularly. I was on a pain pump, where I could press a button and get Dilaudid every few minutes, and I tried to use it as often as I could. This kept the pain under control, but I got occasional twinges from the catheter. I tried to eat breakfast but unfortunately got sick again and ate little the rest of the day but ice cream and yogurt ( I found I could only tolerate soft, sweet stuff.) I was given oral laxatives and daily shots of blood thinning medication every day of my hospital stay, and I also had on anti embolism stockings. Dr. Dugi and some residents came to see me, and he told me all went well. I asked if I could get out of bed, as I had low depth surgery and no skin graft, and he said that would be fine as long as I kept it to a minimum and sat in a comfortable chair by the bed. However, after he had gone, I asked the nursing staff for help getting up and they told me I had to stay in bed and that the doctor had only given permission for me to sit on a toilet chair if I needed a bowel movement. This did not line up with what he told me, but they insisted that was the instructions he had given so I was not allowed to get up after all. By the next day I was very, very sore from being on forced bed rest and felt that I was getting worse in terms of pain. When Dr. Dugi returned the next day I told him I had not been allowed to get out of bed, and he seemed surprised and told me that I could have done so the day before. Long story short, I think there was a miscommunication somewhere and the nurse in charge had misinterpreted his instructions that I be allowed to sit in a chair by the bed as "she can only get up to use the portable toilet." Not the end of the world, but I do think I would have been more comfortable had I been allowed to get up when I asked. That was the only other thing, besides the drain accident, that was less than stellar. Other than that the nursing care was top notch, and I saw Dr. Dugi or at least a few residents every day.

On the third day I was switched to oral pain meds and was allowed to get out of bed, and I started feeling better though I still struggled with nausea. I had a lot of visitors (I just live 5 minutes from the hospital) and was well enough that I was very bored being stuck inside. On Thursday, my fourth day in hospital, the catheter was removed and I was allowed to go home after I had a bowel movement and could urinate. After all the laxatives my tummy was actually pretty upset, so I was able to go go to the bathroom again, and again, and again (in fact I am still feeling them now.) Urination was, and remains, tricky and a big chore due to swelling. I got my first look at my new genitals, and though swollen I was impressed and how "right" they looked and felt. Finally, I was released from hospital and driven the short distance home.  I was given a prescription for oral pain meds, which I have been making liberal use of, and have tried to stay in bed except for visits to the toilet and my twice daily shower to clean off.

I will say this: this is major surgery, even with the "minimal depth" I had done (which actually gave me about 1.5 to 2 inches of depth, as I discovered this morning.) I dread trips to the toilet because urination is difficult (due to swelling) and very messy. I often have to shift position several times to feel like I totally emptied my bladder. Even with powerful narcotic pain meds I am still in a fair amount of pain, enough that sleeping is impacted. I am very tired and randomly fall asleep. I'm really glad I don't have to dilate because I cannot imagine adding that on to what is already a complicated post of regimen. I've had numerous gender surgeries including FFS and my breasts done twice, but this is by far the most painful and the most difficult to recover from. Even so, I am extremely pleased with my decision and with Dr. Dugi in particular. As the swelling goes down I get more and more of an appreciation for the work he did, and I think my healed result is going to look amazing. I already have twinges of sensation in my new clitoral area, so I feel confident I'll be healed up and having orgasms again just as soon as I'm able to. OHSU was great, except for the two minor issues I mentioned, and I feel very lucky that I got to have surgery close to home instead of having to travel elsewhere.

What a great review! Thanks for putting together so much detail for us. I had my surgery (orchiectomy) about fifteen miles from my house, I had some issues during healing, and I was glad I could get back to the surgeon in no time at all.

It's now been 11 days since surgery. Last weekend was rough, with a lot of pain and a LOT of swelling. I had to go in Monday and get the remaining drains removed, and when I was there I asked to see the nurse practitioner because I was concerned about my right labia minora being so swollen. She did an exam and found that I had a hematoma under the labia/clitoral hood, hence the extreme swelling and pain. Fortunately she said I didn't need any treatment besides ice and assured me it would not affect my final result. Since then the swelling has gone down in tiny amounts day by day; the left labia minora is a lot better, but the side with the hematoma remains much more swollen, hard, and painful. I have little or no pain anywhere else so I think without this complication I would be feeling pretty great.

Even with the swelling I've had more time to appreciate just how good the cosmetic result really is. Dr. Dugi is a master when it comes to hiding incisions and doing perfect, tiny stitches. I don't think I've seen any other doctor do such a great job of minimizing visible incisions. I have no visible incisions in my labia; they are all hidden in the creases or in my groin. Like the best doctors now. Dr. Dugi uses groin incisions, but unlike anybody else I've seen they stop right around my urethral area instead of continuing up to my abdomen like I've seen from most other surgeons. This means when I am standing there will be no visible scars, and to see the ones I will have I would have to lay on my back and spread my legs. Another thing I really like is how fleshy/stretchy my labia majora are. In my opinion a lot of surgeons take out way too much tissue in this area and leave it looking flat or stretched. Not so in me. My majora are soft and squishy and according to my husband look and feel like cis women he's been with. My labia minora also look to be well done but it's harder to tell here because of the excessive swelling; right now they are both very firm, with the hematoma one being swollen and hard. I haven't really been able to see my clitoris due to the swelling but I can feel it and it does have sexual sensation. Even though I had the minimal depth procedure I discovered last night that I have a bit more depth than I thought. Urination is doing a lot better but still is a chore thanks to the swelling in my inner labia.

Originally I was going to Dr. Dugi because he was on my insurance and was close to me, but now I am so happy I chose him and would have picked him as a surgeon even had he been far away and I needed to travel. Cosmetically I think my results are on par with the best work of Dr. Bowers or Dr. McGinn, and the hospital stay was great as was the staff (with only a couple of minor exceptions.) I especially want to commend Dr. Dugi's nurse practitioner Kat for being so accessible and helpful when I had concerns. Though it will take months of healing before I can comment on final results or orgasm, I can already say that I highly recommend Dr. Dugi and OHSU. My husband, my male partner, and my platonic girlfriend who have all seen the results say they are very impressed with how natural and all done it looks. In six months or so I have the option to go back for the secondary labiaplasty, but depending on how I heal it might not be necessary.

I had "minimal depth" so I spent only 3 nights, 4 days in hospital though I probably could have done with one more day honestly. They make sure you can pee and have a bowel movement before you go, but due to my minor complication of the hematoma I had urinary issues when I got home and I sort of wish I'd had the catheter longer.

 I have my first orgasm last night in my sleep, three weeks post surgery. I find that to be pretty incredible. I continue to heal well, though the hematoma remains uncomfortable. The cosmetic appearance is pretty amazing even so, and urination is much easier. I am thrilled with Dr. Dugi's work, and I highly recommend him.

It's now 5 weeks since surgery. I continue to heal well, though there is still excessive swelling in the area of my right labia minora where the hematoma was. I suspect I might need a slight cosmetic touch up in this area in six months or so, but it's too soon to know for sure. I cannot really see my clitoris because everything is so tight down there, but I can definitely feel it and it's super sensitive. At about 3.5 weeks in I started having more bleeding and discharge, which at first made me really nervous, along with a pretty bad odor but that has mostly cleared up and I'm told it was due to irritation from the last stitches as well as skin sloughing. I can control my urination fine, but it's still hit or miss in terms of direction (though that is steadily improving) and I feel like I go from "I'm good" to "I need to go pee NOW!" faster than before. Dr. Dugi's office has been great, and I've had multiple post-op visits with them. Right now it's just a waiting game for the next few months as the swelling continues to go down and the incisions calm a bit. I was a bit concerned because when I get aroused I feel a bit "puffy" down there internally, but upon inspection nothing looks abnormal or swollen so I think it's just leftover swelling. In summary, I am VERY happy with Dr. Dugi and his office, and I would recommend him hands-down to anyone I cared about.
