Bone work typically is much more painful than soft-tissue work. The procedures and their sequence are really dependent on each person, and what bothers them the most. Based on my experience, I have listed my procedures ordered by the least pain vs 'gain' (highest reduction of dysphoria):
1 - If you have a prominent Adam's Apple, a trachea shave is a no-brainer. The procedure usually takes less than an hour. Your throat may feel a bit uncomfortable for several weeks, but overall it is not painful.
2 - Brow contouring. Although painful for a few days, smoothing a prominent brow goes a long way towards a more feminine appearance.
3 - I had jaw contouring and genioplasty (chin reduction) performed at the same time. This was the most painful of the surgeries I had (including GCS), but it also produced a noticeable change in my appearance.
4 - Have a receding hairline? A scalp advance can help quite a bit. The only painful part of my procedure came when the nurse tried to remove the drain in my scalp, one of the few times in my life I nearly screamed from pain. The procedure will probably leave your scalp numb for a year or two, so bumping your head will actually be less painful. Side benefits of a scalp advance include a brow and forehead lift.
5 - Thin lips? Lip augmentation can help. It wasn't painful, but it makes drinking a bit awkward for a few weeks. I'm not sure how much of a difference it made in my appearance, but I decided to give it a try. Unlike a fat transfer or fillers, implants are virtually permanent.
