I know the last entry was posted in 2008 but would anyone be able to post a copy of the letter they used? My Therapist was still an Intern at the time that I had seen him, and even though he will be getting it signed off by the Psychologist he was working under he isn't sure how to phrase the letter. I have found lots of information that needs to be included in it but actually writing the letter seems to be going nowhere for me.
