My experience was similar to Hypathia's.  I went into my local SSA office and filled out the application to get a new card, and filled in my new name and marked 'F' on the form.  I only had the court order for my name change, I was still pre-op but the clerk obviously saw me as a woman.  I politely asked him to change the gender marker on my record, said something to the effect that it was a "mistake" that it was listed 'M' on the record, and he said "no problem ma'am" and a few mouse clicks and I was all set!  Took all of 10 mins in NY.  Got my new card in the mail a month later.

Only for birth date and with a court order name change you can get it changed. I only needed a doctor letter to change the gender marker in MS. It does sometime depend on the clerk you go to.

are some of you trying to tell me a therapist letter will work depending on the clerk? if so, i'd be glad to try it in ohio, but my luck with things it usually backfires  :(

Therapist letter?  I don't think so.

Doctor's letter?  Maybe, especially if it is vague about the actual procedure and says something like "irreversible procedure to change sex."  Check on the precise wording required.  I know some who've gotten their endo to write it after they started hormones and it worked.

A therapists letter can work at the DMV depending on your state's rules (mine does).

It depends on the state, I would contact your county clerk's office to see if they have a form to fill out, or if you need to submit your own letter.  I wrote up a petition, and the judge wrote the order for my name change once I submitted all the right papers, however my county was unwilling to grant a gender change because I had not had surgery, and stated so on the order.  However, that was remedied with my new birth certificate post-op.
