I don't know anything about costs for bottom surgery, since I never intend to do anything about that. However, my testosterone costs $60 for 4 months, and that's from Costco, not from mail-order; of course, you have to factor in that you'll have to see the doctor a couple times before getting the initial scrip, and quite possibly will be back in the doctor's office every time you need a refill. If you're anywhere near a low-income clinic and are low-income, you could pay anywhere from nothing to $50 for each visit.
As per top surgery, I hear Brownstein charges $8000; the two I'm looking into, Johnson and Garramone, charge $6000 and $5700, respectively. There are plenty of very qualified top surgeons in the U.S., so you don't have to worry about the prohibitive costs of international travel... and U.S. flight costs have been gradually declining over the past year or two. Overall, we're a lot luckier than MtFs on the cost of transition... without bottom surgery factored in for us, anyway.
