What is the process / timeline on the NHS (UK)?

Can anyone enlighten me as to the process, possibly with some sort of rough timeline for transitioning on the NHS? Including all the steps etc. Like, what do one do at the start? Make an appointment with GP and simply say "I'm a transsexual, please refer me to (who?)."

I'm in London at the moment, so probably looking to be eventually referred to Charing Cross GIC. Looking at their info, they only accept referrer from "local psychiatric or psychological services, which must first assess potential patients". So GP->Psych->GIC ? When and where does HRT come in, and by who? Argh............. ???
