SRS letter from HRT doc

So in his little brochure thing he sends, Dr. Brassard says he requires the typical 2 letters from therapists. But he also requires a letter from the endo who has been overlooking the HRT regimen.

I have already gotten both the therapist letters, but now I'm trying to get the letter from the HRT doc and am having some trouble. At first I went in and asked her if she'll write the letter, to which she replied yes, so long as my blood tests that she just ordered come back good. The tests came back good and I was all set to get the letter, but she never called me back or anything. So I call back after more than a month of waiting and she had apparently written a note saying something to the affect of, "Everything looks good. OK to write the letter."

So, it looks like she is all set to write it, but she's out of the country right now apparently. When I talked to her staff, the lady said that I could write the letter and they'll sign it...so now I'm stuck having to write the letter.

I have no clue what the letter is supposed to say, since the SOC doesn't mention anything (to my knowledge) of what's required for this third letter.

I guess my point is, have any of you had to get this third letter, and if so what did it say? If you have a copy of it would you mind if I use it as a template for the letter to give to my doc? Also, if you've gone with Dr. Brassard as well, did you end up getting this third letter? Is it necessarily required, or is it just something he'd like to have that isn't required? I'm so confused about this whole other letter.

Hey Sarah, 

I went to Montreal last summer and I don't believe my hormone doc wrote a letter to them.  I had to fill out a Medical History document, but I don't think my doctor even looked at it before I sent it.  Of course, I wrote his name and contact info on the form if they needed to get in touch with him. 

I would ask them if it's OK to send the Medical History document (maybe this is what they are referring to) and have the doc sign it.  If that's not good enough, just type up something like Ms. Sarah R___ has been a patient of mine since mm/dd/yyyy and has undergone hormone therapy in accordance with WPATH SOC... you get the idea, put some big words in there and have the doc sign the bottom.

Good luck on your surgery, starting to get the butterflies yet?  You made a good choice, they will take good care of you up there.

Hey, thanks for the advice. But I think they mention both the medical history form (which I already returned to them) as well as the third letter from the HRT doc. However, if you say you were able to go through the surgery without the third letter then maybe they don't care so much about it. I'll see what I can come up with and have the doc sign it.

Actually, I'm not worried at all. No butterflies. I'm just getting impatient at this point since it's so close.

This is very similar to the one my endocrinologist sent to my surgeon.  All the best for the future and hope you have a speedy recovery.
