1) I haven't worried about that for a long time. After a while you stop thinking about it and it's fine.
2) if telling a medical doctor will mean I get the proper treatment then I don't care.
3) I rarely wear pantyhose, but do occassionally go au natural under my skirt. Not advisable in a sarong however.
4) after surgery, yes it won't be a problem. I feel totally comfortable in public restrooms now. And btw, it isn't my gender of "choice", it's just the gender I am.
Whether someone knows, or guesses, or whatever, really isn't my concern. I steadfastly refuse to hide who I am from anyone for any reason. Everyone just accepts me as the female I am and I have had no problems, not even when flying internationally and I've done that quite a lot since beginning transition. It's no big deal.
