insurance question.

I know there is information on here somewhere and I hate to beat this topic to death; but in the last few months some companies include transition coverage to, varying degrees. My question would be..is there A company that encompasses the surgical portion of SRS,vagioplasty, labioplasty, ect. for the individual ( not a company based one) and how ,with the recent court rulings,does medicare fit into the picture? Any help would be appreciated, thanks.  Jolene
