My name will be Jacob and I'm going by Jay as well.  :) I like Jacob Riley Parker as a name, too. I'm too scared of privacy concerns to post my middle name here though.

I've already changed my last name, so I don't really want to do that again, I like the one I chose, it's the only thing I really truly liked and am comfortable with.
