you totally get what needs to be done, don't doubt yourself. i have seen lots of the surgeons now and i do find that they can make you doubt your original opinion and confuse you! but you have a great understanding of your face.
i reckon the most dramatic change for you will be your forehead (and full setback would be most effective as opposed to just a shave). the projection overshadows your pretty eyes and also creates a harsh angle from the side and 3/4 view. pulling that back and opening up your eyes will transform your face. everything else will be the icing on the cake, but not essential. and lucky for you your hairline is nice and low so you could even avoid a visible scar with an incision behind the hairline.
certainly refining your nose would have a positive effect - some deprojection at the tip end and slight elevation of tip (but not a piggy nose of course

)
OMG DO NOT TOUCH YOUR JAW! I do like Dr Z but that is absolute craziness. you are totally right in your instinct, ignore it and cross it off your list.
Your chin projection is lovely from the side and 3/4, it just could do with a touch of narrowing from the front and a slight vertical reduction.
Your lips are lovely. please don't pump them full of fat, they are feminine and natural and just don't need it. they will not look good. a lip lift could enhance things, but it isn't essential and remember it will provide a scar, so don't rush into it. you could always do it later if you felt it was bothering you. don't let surgeons trivialize these kinds of procedures, throwing them in there casually. every one has its risks and potential complications so the fewer you can get away with the better

before committing to fat injections into the cheeks i would test the water with fillers to see if you like the result as at least they are temporary. Adding volume there by whatever method might help widen the cheeks and therefore balance out the narrowness you mentioned in your jaw, creating some curves there, but there is a risk of overstuffing because you do have quite a narrow face, and so i would proceed conservatively. However i do think that fat grafting to the temples would be very beneficial. Indeed if you could round out that area i think you would see less of a need to do anything much to the cheek area.
surgeons like to encourage you to do a one-stop shop and get it all one in one sitting, but i don't agree. they will say it saves you money etc, but the money you would save from not splitting any additionally procedures such as lip lift and fat grafting or whatever is nothing if you think about these things affecting you for the rest of your life. and as i said before, you don't want to be doing procedures you don't need, and sometimes the only way of knowing is by doing the more obvious things first and then sitting back and assessing things. You can never go wrong that way, even if it does take a little longer to achieve your ultimate goal.
so in summary: forehead setback definitely and unreservedly, along with chin and nose and fat grafting to temples. anything else i would hold off on for now.
have you consulted any other surgeons apart from dr z yet?
