I had BA in Feb 2006 after 8 years of HRT. I stepped it up from a small A cup to a full C. Mine are saline filled, under the muscle. First let me just put it out there.... it hurt like bloody hell! The way I've described it to others is that it felt as though I got shot in the chest with a cannonball. The first few days I could barely move. It was all I could do to get up and pee. The pain meds are all that held me together for those first few days. The healing process took some time. For a few weeks I couldn't lift more than 15 or 20 pounds. I had a lot of difficulty closing a sliding glass door for a while; something about the muscles involved to pull the door... well, I was very weak for a while. My car has a hatchback door instead of a regular trunk, and that door takes a bit of effort to pull down to close... I could not close the hatch for what seemed like 3 weeks to a month or more. Literally, I would be in a parking lot looking around for a stranger to help me close the hatch on my car. Eventually it got better, I got stronger, I survived. But OMG did that hurt!
That said, it was SO WORTH IT! I'm sure that the doctor told you that it will seem like the implants are just under your chin at first. I was alarmed in those first weeks because my boobs were sitting up so high. They gradually settled down into perfect alignment. Honestly, it seemed like it took six months or a year before I felt like they truly settled perfectly. They were in pretty good position after about 6 weeks, but they very slowly got better and better over about a year. My boobs did continue to grow over the years; I'm presently at DD, and I'm not heavy or fat (my waist is 26 inches, my boobs are DD... a little bit top heavy, but I get a lot of attention.)
I love my boobs!!! I'm glad that I didn't go any bigger. I would totally do it again! My body is hot. My boobs are amazing! My back is totally fine. (Of course, I do have a desk job, and I sit on a big inflated ball instead of a chair all day everyday; this keeps my back straight and my core muscles strong. I've been using balance balls in place of desk chairs at work for 11 year now. Strong core muscles will prevent the kind of back pain that some experience from big boobs. Just sayin'.)
