I would think it'd be best to start transitioning abroad and simply lie to his parents about the diagnosis if he doesn't have one of these conditions. I would say this is better than coming out as gay.

There's so many different dialects it's almost impossible to get it to stick unless you're in the country using it (or at least hearing it) daily.
