Two words for you: Häagen-Dazs®
Unless she's like me. I have always been skinny and always will be, I can smash burgers like no other and eat piles of fries. I may have a full looking tummy for maybe 2 hours and then back to skinny. My parents were told by doctors to take me to Carls' JR. McDonald's etc, they told them to put me on weight gaining shakes and protein supplements etc. All it did was make me want to gag, I hated those shakes!! Never gained an ounce btw lol.
Doctor's said I'd gain weight once I was 30 + years. Which is true I gained about 10 lbs since I was in High School. I'm almost 6'2" and barely 135 lbs my last visit, and that was with shoes on lol.
My point in all this is I know what it is like to be a "hard gainer" which is the technical word for it in the fitness world. When I was on HRT at 22/23 I did see results in the breast area quickly and had I not stopped I think I'd have gotten at least a good B. At 35 I have a little layer of what I think has to be fat, as I don't have any chest muscles (never have). I also think because of that and my first stint with HRT I'll probably see pretty quick results.
I'd say at your age you should definitely see better results, 2 years is awhile but I don't think being thin weighs too heavily on it. My sisters and mom are skinny too (runs on my moms side) and my mom has D's and my sister has very large C's. So at least in my case I'd say it weighed more on genetics than on weight. I'm not an expert though and very new here, so just talking from my perspective and experience!
Good luck, I hope you see some changes soon!
