I'm American. Blue Cross Blue Shield appears to cover it, but I can't tell. From my understanding, most companies only have trans policies that cover trans related surgeries only through employers and not through individual policies. I know cigna works this way.

I don't think any insurance company (a business designed to turn a profit) wants to take a couple hundred dollars as your first payment, give you an expensive procedure, then have you cancel. There will be a contract involved, and you will be bound to the terms of the contract. Hugs, Devlyn

In the US insurance is generally thru the employer. I doubt BCBS would be amused re: expensive procedure and a cancellation.

Preexisting conditions are no longer allowed for insurance determination. I would suggest simply calling insurance companies and asking them the cost of a policy that would  cover the surgery. It is not unusual to hear of $3,000 deductible. You will already have half the surgery paid even before you pay your first premium. I would focus on stashing the money away. You also absolutely need to get a larger binder. If you are hurting that much, it is clearly too small. Take care of yourself by going up a size.
