Got a draft of my HRT letter, comments, suggestions?

My therapist just sent me the draft of my HRT letter for comments and corrections. It's the first one she's ever written since I'm her first transgendered client. I'm including it below with identifying info redacted. I know she refers to me as "he", I'm OK with that at the moment as I present as male. What do you think?

I am a clinical social worker in private practice and
I am writing to you on behalf of my client <redacted>
who consulted me for Gender Dysphoria.


<Redacted> is a 55 year old transgender person who has felt out of sync
with his own physiological presentation throughout his life.
He is eagerly seeking an appointment from an endocrinologist
skilled and trained in working with transgender persons.

<Redacted> has been married for 33 years. His spouse is now aware
of his identification since therapy was started with me.
The <redacted>'s are parents of a 29 year old who is pursuing a career in medicine.
<Redacted> told his son that he is transgender.

<Redacted> feels ready for hormone replacement therapy.
He has done a significant amount of research and  has carefully pondered the anticipated consequences.
He wants and seeks medical advice at the earliest possible time.
April 22, 2015, the day of my first face to face pass in gender neutral clothes and no makeup. It may be months to the next one, but I'm good with that!

Being transgender is just a phase. It hardly ever starts before conception and always ends promptly at death.

They say the light at the end of the tunnel is an oncoming train. I say, climb aboard!

Most also will add the WPATH SOC knowledge to there writings.
