Hi everyone.  I'm hoping some Canadian girls can help me out a bit. I went through CAMH and am waiting to set a date for surgery with Dr. Brassard... just waiting on that funding letter!  If i understand correctly, camh will send all the letters to Dr. Brassard in Montreal.  What do I need to send him?  Just the medical questionaire and photo?  I'm sure this seems really simple, but I don't want to screw things up.  Thank you!

Call them. Avoid any confusion. Your situation might be slightly different or rules may have changed.

Yes you need to do the information paperwork and photo,

Just keep in contact with Suzanne in the office. Once she gets the ok for the payment they will send a email on the date if it is ok.

Then they will let you know if there is any payment. I know some girls from other Providences have had to pay for some medications. I have now idea why and it is personal so i never asked them.

So get in touch with the office so they know to expect camh will be sending paperwork on payment.
