Well I found Fort Lauderdale Florida very trans friendly. I think pretty much it's a blue state there. Going from here in NM that's saying something I suppose. There's a guy in Washington or Oregon, I could find his name if you are interested. He's not as well known, and don't know that he'd be cheaper. He did a rather complex job I saw that looked good.
I don't think Garramone is expensive as top surgery goes but he is likely to charge you full price, I think. (Depending, unless it's not too complicated.)

Yeah, I'm not sure where this idea that Garramone is expensive came from. He is actually more the mid-low end of cost, with the whole kit 'n caboodle costing $6,900. I haven't read/heard of any top surgeon's services, plus hospital fees, etc., being below $7,000, but I certainly could be wrong.
I suppose some plastic surgeons who don't specialize in top surgery may have lower prices (since it isn't their specialty), but I don't even recall ever reading/hearing that.
Felix, the main thing I would suggest is if you are seeking revision, go to a person who specializes in gender reassignment surgeries, not just a plastic surgeon who has only done a handful of top surgeries. It seems to me a revision could/would be more complicated than working from a clean canvas, so to speak.
