Or maybe not... What really sets this one off is her use of flare in the skirt. She does have some crazy nice proportions (almost seems impossible), but the fashion choice is what is greatly amplifying the effect. Turns out, empire waistlines are an amazing style for MtF's- one of the best. For a lot of us who have gone through a male puberty, it creates fullness in the hips extremely well, and especially shows off a slim waistline.
Wild Flower, I would say forget about the leg bone surgeries. Men and women come in all shapes and sizes, and I'm sure that you overlap the two (just like everyone else). If you really can't live without doing the most you can in the hip region (I put myself in this category of desire), about the only thing that is reasonable is fat transfer to the hips. That is, of course, on top of HRT- which also does an incredible amount of body feminization naturally on its own.
The bottom line is that it is an idealized shape that is most likely unrealistic to dream of unless one were to start hormones in the pre-teens. Even then, with female puberty, having a figure like that (without corseting) is still totally idealized. Basically what I'm getting at is that it's not really worth considering a passing issue, as other's have mentioned. I guess it might help, yeah, but it's totally not needed. There is SO much you can do with fashion- especially with HRT.
