There are some links on this site to FFS surgeons that might be in your area or state.  I live in Australia so I am currently looking at going to Thailand to see Dr Supporn.  I sent an email to his office and also to Dr Speigel and received an email back from both of them within 24 hrs.  They requested I email pictures of my face, at 5 different angles.  They then can see what needs to be done and give me a quote.
There are many other Doctors who specialize in this field so maybe others will reply and give you their recommendations.  I really like Dr Speigel, but the accommodation and travel over to the states would be out of my league unfortunately.
You can check out these sites and heaps of others that can give you a step by step guide of what to do, but it's ultimately your decision with who you choose.
