The past few meetings with my grandmother has little snippets of comments, " you have pretty hands, prettier than mine". "You have small ear lobes for a guy", and she even said my hands were smaller than hers.

She loves me, probably more than anyone, but do you think this is her way of accepting me? She never calls me names or accusations even when my mother brought to her every blue moon or so.

Are you out to her ?

Well some people are made for this...
a lot of FTM people for example have high testo levels...
and some MTF people have favourable features already 

I'd say be happy...

She is probably the only person in my family who loves me unconditionally and puts up with some of my weirdness.

I was kind of a wreck today because shes not all that healthy and I will one day lose her. Im just trying to make the best of the present because at least shes there for me on this lonely world.

You might look for some support groups...
for example with lgbt center...
or meetup groups...
and there is also a chat on susans...
