What age can I get my Birth and Middle name and gender changed?

I'm just wondering cause I don't want to be called Female Pronouns and my birth names. Also is there any like things you need for approval to allow your name and gender to be changed? Like do you need Top Surgery and Hysterectomy and Lower Surgery to assure them your now male? Also is there a certain age that your allowed to do so? If there isn't do you just need parent permission if your under 18? Also are you able to change your last name in the process also? Just wondering cause I don't really like my last name because of my family. Sorry if its like stuck up. One more thing, Can you change your name to someone that you know who has died. Just wondering cause I have an Idol named Petrus Thomas Ratajczyk (Stage Name: Peter Steele) Or would this be like theft. He's been dead for 6 years now and I'm not at all trying to take his identity away but I'd like to have his name cause it's very unique and a god like name. I'm sorry if this is very ignorant..I feel like I'll get haters for this (I've asked on many wrong sites)

If you are 18, you can change your name now. If you are under 18, you can probably do it now with a parent's permission. You can change your entire name if you want. There is no requirement as to a reason for changing your name, but depending on where you live you may have to speak to a judge or publish it in the paper.

Certain states require that you have SRS before they will update your gender marker. It varies. The link Devlyn posted should help, and if it doesn't, let me know and I'll fix it :)

You can certainly change your name to someone else's, but I personally think it would be distasteful to take a dead celebrity's name. Even though you can technically change your name as many times as you want, it costs money and you have to update all your documents every time. I would think long and hard about what you could live with being called for the rest of your life.

In my state you need parental permission if you're under 18. You can have the same name as him, I don't suggest it however. I don't suggest changing your last name even if you hate your family right now. Gender marker for birth certificate and license/permit/state ID depends on your state. social security and your passport, I believe they only need a letter form a doctor saying you identify as male and are undergoing transition, it doesn't have to be specific as to what type of transition.
