Hello, I am 26 and have been on hormones for 4.5 years. Here are some photos of me, I just had my second hair transplant done so forgive me for how my hair looks.

What do you think Dr. Spiegel will recommend and what would you recommend? My hair transplant surgeon warned me against getting forehead work done too quickly as it could affect the hair grafts from my hair transplant. But I want to get this done as soon as possible, dysphoria is killing me.

I personally would recommend forehead work (it would have been better to first do that and then use the transplants to cover up the scar line from that). I also would recommend loosing some weight, as the blurring of the jawline makes the face appear more masculine or androgynous. An Option would also be to add jaw work, but I think it is not needed neccesarily. I think the nose is ok, probably it would also even fit a reshaped forehead.

I had FFS with him on 9/16.

Upper lip reduction with fat injection.
Fat injection of temples and cheeks.
Forehead work.

I can not judge the mandible and chin.

Hi.  Oh my.  I'll be as honest as I can.  You really should wait until those recent hair grafts mature.  But if you can't, you might want to opt for an ear-to-ear coronal incision for your forehead recontouring work so that the scar is hidden in the scalp away from the recent hair grafts.  That will, however, cause your hairline to recede somewhat depending on how much skin has to be removed from your forehead requiring more hair grafts to bring it back down.  Actually, that wouldn't be bad because whoever did your hair grafts should have made the hairline somewhat irregular (rather than so smooth) to make it look more natural.  Other than that rhinoplasty is needed to feminize your nose along with an upper lip lift.  I would skip the jaw work.  It's hard to tell if you need it or not, but I think it's okay as is.  It doesn't look like you need a trachea shave either, but if you plan to lose weight, it might become apparent. 

Have you considered other FFS surgeons besides Spiegel?  I had mine done in Marbella, Spain by Facial Team.  I highly recommend them.  Cost less, too even with extra travel expenses.

Welcome to Susan's Place. I am not really all that knowledgable about FFS and in this case, I am not sure what a doctor would suggest. I think what you should do is take the word of your transplant doctor and set a date when the transplant can withstand the surgery. I suspect with the wait list the doctors has, then next available window may be late enough that the transplant will not be an issue.

I'll chime in here as well and say , wait at least a year before getting forehead work done after new grafts. The trauma of cutting at / around the hairline will already potentially cause issues with those hairs but if they're still new, it may end up killing them off completely and they won't actually take.

So I went to Dr. Spiegel on Wednesday and scheduled my FFS for August 1st, 2017. I'm getting my forehead done, a rhinoplasty, a lip lift, cheek fat grafts, and mandible contouring. My hair transplant surgeon mirrored the concerns about waiting a year before surgery, so I'm going to wait about 10 months.
