I wouldn't know any US surgeons who can help you. But as Max said, you will need both a plastic surgeon and a gynaecologist.
Personally, I'd say, by all means go ahead and combine the two. Here in The Netherlands, I'd say about half the guys get those two together and healingtime doesn't seem to take longer than a seperate chestsurgery. It mostly depends on how the person himself heals. (and I'd say the main reason it's 'only' half and not more, is because doing chest first would mean a shorter waiting time and since we have to be a year on hormones before we can have any surgery, we hate waiting even longer

) Following the procedures here, the only reason I wouldn't recommend doing both at the same time is because we have the possibility of combining the hysto with the colpectomy/vaginectomy, this greatly reduced the chances for complications with the colpectomy.
As most surgeons outside of The Netherlands that perform a colpectomy do this in the same surgery as the phalloplasty, combining chest and hysto wouldn't be a problem at all.
