I'm in a position where GRS is finally a realistic option for me and I'm now trying to find the appropriate surgeon.  The problem I'm having is that I'm not aware of anyone who has my unusual situation regarding sensitivity.

So I've never really had sensitivity in my glans and it's been incredibly rare for me to respond to any stimulation to it; far less for me to reach orgasm through it being stimulated.  The area which is sensitive and which has eventually been what's brought me to climax functions more or less like a g-spot; and it's located along the urethra; however this is not close to the prostate either as it is in front of/above the scrotal sack; rather than behind it.  And this was the case both prior to hrt; and since my testosterone has been suppressed and i have been on oestradiol and progesterone (although orgasms are a lot more full-body now).  I am circumcised which may be the reason for the lack of glans sensitivity; but equally part of me wonders if this weird mapping of erogenous ones may itself be an expression of dysphoria.

And basically I'm wondering if anyone has or knows of anyone who has or had similar; and if anyone knows who the best surgeon to work on this would be.  I both am in need of vaginoplasty (I experience dysphoria from my current set-up and a vaginal cavity is needed to correct this) but also do not wish to lose sensitivity.  And most of the biggest name surgeons obviously focus on the glans and constructing a sensate clitoris from it (I'm aware this is the focus of the Thai surgeons in particular).  Any advice on who would be best to approach would be very much appreciated.  I have the option of having GRS performed by Mr Bellringer here in the UK if I wish it to be, but would prefer to seek the services of a surgeon who would be more able to work with this specific and unusual need.  I am willing to consider more or less any surgeon if they have the skills to do this; so price is not my main motivation; though whether I could manage to wait another 3+ years for someone like Dr Bowers, I am not sure of.

This seems like a very personal and rare instance.  It's great you have been so self-aware to figure this out and be able to vocalize and pinpoint where your major innervation currently is. Lots of folks here on this site will probably have advice.  I saw Dr Meltzer who I think is very good and thoughtful on where to retain and place nerves.  In my case he maintained the obvious glans and created my clitoris which is well hooded since I was not circumcised and had a very very over sensitive glans but he also placed additional branches of the residual nerves under my mons which are now the area I'm orgasmic through that region mostly rather than my clitoris.

My advice would be to write all the top surgeons and explain your situation and ask them to contact you to talk about their experience, if they have dealt with anything similar and how they approached it and how were the outcomes. You could also ask if they would be willing to ask those patients if they might be willing to talk to you.  This is a very important issue for your long term health. Any surgeon worth your consideration and money should be willing to discuss and talk through your case with care - try talking to as many as possible before making your decision.  Good luck - these surgeons are I think some of the best and do care about us and don't just consider us $$$. (With an odd exception).

I've sent specific questions around this to the offices of Dr Bowers and to Dr McGinn, as well as to Mr Bellringer in this country; I'll probably write to a number of the other highest thought of surgeons or those Bowers has been involved in training given her reputation (Meltzer, Brassard, Satterwhite, Wittenberg, Ting; Suporn, Preecha, Chettawut) but obviously it would help me the most if I can find someone with direct experience of similar because I'm aware many surgeons may promise to meet my needs, but at the end of the day if they damage or dispose of this tissue then I don't get a second shot at it.

I don't know if this helps, but Suporn's got a reputation for keeping as much material as possible - so much so that sometimes there's too much you you need to go back for a revision to remove some of it. But that's also the advantage that he can fix stuff afterwards since you can always take away, but not add back. I don't know if this means much in your case, since I'd assume just having it doesn't mean you'll be able to feel it.

He also has a secondary sensate organ, or Chonburi organ as he calls it. If you can't feel it now, perhaps that won't make any difference.

So far as I'm aware, Suporn uses the glans to construct both the clitoris and the "chonburi organ"; with penile skin used in the construction of the labia minora
