Type III. To me that calls for forehead reconstruction, not just burring down the brow bone. It was a major consideration for me in selecting a surgeon to correct my bossing. Many surgeons with whom I consulted said they can do Type III foreheads without osteotomy (removing and setting back the anterior wall of the frontal sinus) the way Drs. Spiegel (Boston), Demaggio (Buenas Aires), Deschamps-Braly (San Francisco), and Facial Team (Marbella) do. Some I talked to said they do not X-ray or CT scan the forehead beforehand to see if the brow bone is thick enough to get by with burring alone. Those who burr the bone go as far as they can and stop before perforation even if it means not removing all of the bossing present. Then they use bone filler to round out and smooth the forehead. Too often the patient ends up with a less than ideal forehead contour.
I selected a surgeon who did a full forehead reconstruction, and I'm so glad I did. The CT scan shows a thick brow bone covering the frontal sinus cavity on the right side, but a thin bone on the left side. Had I settled for burring alone, I would not have achieved the forehead contour I did.
