I have decided to go on a forehead burring without a sinus setback.
I have a bossing brow ridge. My doctor will burr 4mm from the 'highest point' on the brow bone and will add some forhead augmentation on the top.
The procudure will be endoscopic.
Has anyone done it or seen results by this method?
I have decided endoscopic one mostly because of the unsees scars - 2 in the eyebrows, and one in the scalp.

I had a type III brow ridge reduction. You are so fortunate if all you need is a brow ridge shave. I always recommend to get the least amount of surgery that is necessary. To often to much is to obvious.

I did not have a Type II forehead contouring, which sounds like what you are planning to do. I had Type I endoscopic brow contouring done by Dr. Zukowski. I think it turned out well, but I did not have a pronounced brow ridge. Either way, Dr. Zukowski made a good case for not doing an osteotomy and opening up the sinus cavity if you can avoid it. You risk a lifetime of sinus problems.  As you point out, an endoscopic procedure also avoids the scarring and numbness from a coronal or hairline incision. If you can get the aesthetics you want going this route, I would definitely recommend it.

For very prominent foreheads with large frontal sinus cavities, the most possible reduction without making a hole in the forehead is about 2mm especially in between the brows.  Multiple studies of skulls have shown the anterior sinus wall is only on average 2-3mm in thickness.  Burring can be very aggressive in the lateral brow because the bone is much thicker.  If you have a very prominent forehead and large frontal sinus cavity, I would strongly advise against an endoscopic approach and consider the Type 3 Cranioplasty where you can easily set back the forehead 10-15mm without compromising the integrity and strength of the forehead.  We routinely perform revision forehead surgeries on patients who have either undergone an endoscopic or burring only technique elsewhere. Its just not possible to setback the forehead maximally without a Type 3 Cranioplasty.  This is not a surgery you want revised or repeated.  Scarring is minimal and well hidden with an open Type 3 Approach.  The incision can be completely hidden if performed behind the hairline.
