Hi ladies, I've had forehead recon (which wasn't perfect, but improved my appearance) and a nose job. But, and it's a big one... I am left with this long face which still either gets me clocked or just makes me so unattractive that it literally turns heads (in a bad way).

I'm in IL and am looking at MDs who perform Lefort I, II, III. I will travel anywhere. I personally think this is as important as my forehead work.  It I'm wondering why none of the FFS surgeons I've seen in the US have offered it. Is it cuz it's not typically done by the fFS'ers and they're afraid to mention it cuz they can't do it?

Any recommendations on surgeons or anyone else had this done? I won't consider the four most common fFS guys in the US as I've had consults in person with them and not one mentioned it. Even the two who do that surgery. So I'm looking for an oral/maxiofacial md/DDS person.

Ok.  Hairline incisions are usually used to advance the hairline downward somewhat.  It doesn't sound like that was your result.

Sorry Maddy, I don't think I follow. The problem is with my mid and lower face where it's very flat, no definition and my lower jaw is too far behind my upper. I have a receding chin and it looks awful. In short, I have a long, no definition face. Someone said I look like a madglianai painting. He mad his subjects have an elongated face. I was like, um thanks for the compliment, bud!

I dont know about the US. Internationally, it seems Koreans are really really well known for this tpe of surgery as an elongated face is a common issue among Koreans - probably its an asian thing that it is part of the genetics but it is considered to not be beautiful, so they do a lot of these. ID Hospital and others in Seoul are offering it

We tend to be our worse critics.  I too am worried how FFS (mostly upper) will affect my proportions.  In the end I think that there is only so much we can (or should) do.  The relentless pursuit of aesthetic perfection is a trap, and one of the deepest, honestly.

I agree with Maddy, we do tend to be our own worst critics. I used to be very self-conscious of my sharp jaw line until someone showed me some well-known cis-gender women who have a sharp jaw line.

The difference between what makes sense to change and what not is hard to find. Basically you will find cis women who have each one of the features that we regard as typical male or masculine and want to get rid of  - even facial hair. But those cis women who have one such feature seem to be unaffected by it in their gender perception. My guess is always, that it is the sum of it - one or two such features in an otherwise all feminine face are interesting, remarkable etc - 5 or 6 features are considered trans or get you misgendered. And not all features are included in the usual lists about FFS - in part because they are not yet considered as facial gender features or because they are impossible or very hard to change. Like an elongated face for example - it is rarely described as a gender marker and the surgery to change it is quite invasive, it seems. So it may pay off to rather change enough other, easier changeable features to fix the sum, the overall gender perception

It sounds like you have more of an orthodontic problem rather than just a long face, especially if your upper and lower front teeth either completely overlap when you close or the upper ones are way in front of the lower ones.

I suggest that you find a very good dental team to have a look.  Lefort impactions and mandibular resections need to be done in conjunction with orthodontics and not lightly entered into.   There are more conservative options available but will depend on your situation

Yeh if the issue is your teeth/jaw alignment then a maxilla facial surgeon is who you need to look for. They can cut and slide the lower jaw to align the upper and lower teeth first. Some will do in depth reconstruction surgeries so they might do everything. 20 some years ago they would break the jawbone and reset it wiring the mouth shut for a time. These days they use surgical bone cuts.

Some FFS surgeons will cut and slide just the lower chin portion or add implants. That would be if your jaw is already lined up.
