From what I have gathered on the forum people generally wait around 2 years before FFS.

I had my facial surgery 10 months after starting HRT, and would have had it sooner if it had been practical.

HRT will not change the underlying bony structures of your face.  It can help with complexion and skin quality, and may even change fat distribution, but things like a brow ridge, overly large nose, square jaw and chin, none of that will change without facial work.

I knew when I started transition that facial surgery was one of my top priorities (along with beard removal and voice retraining -- and again, HRT doesn't really help with those, either) given the shape of my face back then, despite being passable.  But I did all that because my dysphoria was such that I could not abide any kind of misgendering; I refuse to be clocked (which is also why I practice non-disclosure).  And it was so important to just how I saw myself, too.
What you look forward to has already come, but you do not recognize it.

I think it makes sense to wait for 2 years on a working good HRT - not low dosage menopausal stuff as some doctors try to do but something that really makes a visible change. If you need FFS earlier to be rid of dysphoria or to "pass" because you already are "presenting feminine" to everyone but are having trouble there, there is no reason why not to do it earlier.

Waiting 2 years or more has 2 advantages - for once, maybe you are passing and feel happy without FFS because of hormonal changes - usually this can happen if your hace fas underlying neutral bone structures. And the other thing is that with HRT having done some work, the surgeon can easier guesstimate what changes make sense. Comparable to a breast augmentation - if done too early, the own breast growth later may be so much that the breasts end up too large to be comfortable - if you get cheek implants now in FFS and after 2 or 4 years of HRT the cheeks fill up naturally, it may look fake.

I think it will vary, like everything, from person to person but I waited 19 months before I had my first sit down consult with the surgeon. I had my consult last month (and half way through laser hair removal) only because I believed they had a 6 month waiting period between consult and surgery (I am local so no real wait). I was/am going off the 24 months for changes to set in and full facial hair removed. I thought not having a 5 o'clock shadow may matter, and honestly now that it is gone it does matter to me a little bit (I don't hate my jawline any more). Of course my huge brow ridge and ultra square chin were not going to change but I thought it best to let HRT do it's thing, get rid of the facial hair, get settled mentally, and be patient. To be honest though when I started HRT I thought I needed and wanted everything to happen RIGHT NOW but luckily for me patience, planning, and preparation were not so difficult when the hormones started.

I agree... There is no reason to wait for for HRT effects to have boney FFS... Before starting HRT is fine...

That said if one does have FFS before or very early after starting HRT, it would likely be  a good idea to wait a couple of years after boney FFS  before having facial soft tissue work such as a facelift... You might find out you don't need it.

I never had a facelift after my FFS... in my v=case I had FFS about 2.5 years after starting HRT ... Once I healed and all the all the swelling went away, it was not really needed back then.

HI,
I waited for around 10 months. I started talking  to Doctors around 6 months. I believe it was the best decision of my life. Within in 3 weeks my life completely changed. It can make that big of a difference if you go the the right doctor for you. You have to do what's right for you. Me personally I didn't want to waste any more time being mis gendered. It does work for some people to wait. But you can't change you bone structure. Best of luck on your decision.

I'm having mine in one month, at that point I'll be around 19mo on hrt. Like another user said getting facial hair removed first makes a huge difference in how a face is perceived.

The biggest change I noticed when on HRT was my neck became slimmer. While I did not notice it at the time, there was definitely a difference looking back in photos. As for the rest, wrinkles came faster & my hair line actually receded. So did not experience the bliss most speak of.

While I am now seeking a new endo & of course obtaining insurance, as mentioned, HRT will not fix bone structure. Facial hair removal does influence appearance. But if one is dysphoric about a nose, brow, chin etc. Am of the mindset to research FFS surgeons & get in line with them. Some have drastic long schedules to get on their table.

I effectively started HRT at about 15 months prior to FFS.

I would suggest that regardless of hrt time, If I could do it again I'd have electrolysis finished first.

I will have waited 11 months for FFS surgery after starting HRT. Electrolysis was started a year prior to surgery with 3 hours once a week. Very happy it will almost be done before surgery.

I also have to agree HRT effects soft tissue not bone structure and the only time bone structure can change with HRT is at a young age, prior to puberty.

Saying this everyone is different so a person with more fat to distribute should wait longer to see changes. Im afraid Im very small with not to much meat at all so very littler soft tissue changes.
