I've been in a constant state of ketosis from day one of hrt. No adverse affects on hormone levels, feminization, fat redistribution or breast development.
I also have been intermittent fasting for over a year now on a daily basis.
From the research I could find of girls going through puberty with low BMIs it appears that if there is not a lot of fat for breast growth, the timeline just extends and simply takes longer rather than being stunted.
I also hypothesize that if you are combining keto with IF one may benefit from the increased levels of hgh, which of course are highest during puberty.
I am just over a year into full transition levels, with six months before that of low dose hrt, slowly titrated over time due to the slight risk of seizure activity, also the reason I'm on the keto diet.
Needless to say fat redistribution is going well, breast growth is coming along, close to a full 34 b cup now- yay! That's with about 115 lbs of weight loss during that period. So obviously weight loss + keto did not impair breast growth for me.
