I'd encourage you to open up a conversation about this with your surgeon, as some are more strict than others. Your circumstances are obviously different than perhaps someone who worked in an office all day, and your surgeon should have some recommendations or compromises to help you.
It's also personally reasonable to ask politely if they could refrain from smoking while you're in their apartments due to health/medical reasons. It is accurate and true, but it's also a topic that most people will respect your privacy on and not dig further. There are countless medical reasons to need to avoid secondhand smoke, so it wouldn't raise many flags.
