Welcome to Susan's Place. There are several procedures that can correct this problem and several doctors who preform the surgery. I believe some of the Thailand surgeons do revisions and I know the United States and India also have surgeons who are able to correct a shallow vagina. Normally surgeons use the colon to provide the donor material so with this approach, depth is limited only by the space available in your body.

As I don't know the original reason for the surgery being shallow, one possible suggestion is you must place at least 4 pounds (2 kilos) of pressure on the dilator in order to reach sufficient depth when dilation. While the gel can help convert the skin type, I suspect it will do little to improve the depth.

Things that you should read

Under a doctors direction, estrogen is safe to use. Without medical monitoring, you risk DVTs and liver damage. Normally the risk of this is very low however people with a genetic risk of DVTs or a damaged liver run a much higher risk. I was given the gel after surgery to help convert the skin of the vagina to vaginal tissue much faster than the oral dose I was on.

There is also a risk of calcium loss in your bones if you don't use some form of estrogen supplement. In addition, because of outdated medical advise, I was off estrogen for around 10 years and lost a fair amount of feminine fat distribution. A lower dosage of estrogen is required post surgical just to maintain your gains and maintain bone health.

So the gel that you use you also put it directly to your canal?

I have alway had HRT in pill form. There are several that come with and applicator and are intended for vaginal usage. It was provide after I healed from surgery and I would insert it before my last dilation of the day. My gynecologist prescribed it when it was determined that I would benefit from using it. The intention was to provide high estrogen levels in the vaginal area so the skin would feminize at a rapid rate. I only used it for a short time and I'm not using it now.

Estrogen jell for HRT is something we tend not to use much in this country. There is no reason why it wouldn't work well but we have so many other options that I have heard of only a few people who use it.

I think you will find that people are generally helpful on this site. If you don't get responses, normally it's because of the way your post was written. Try to provide as much information as you can and clearly state your questions. Sometimes it may take a few days for the right person to see your post but normally you will get a response.

Our foundation researched this and advise our sponsored applicants that the depth the surgeon obtains in the first dilation at their clinic will be the depth you can expect and need to maintain. Depending on the surgeon they will have recommended a dilation schedule.

The first few months dilation is very important and should be almost the maximum pressure you can apply using your middle finger. Its around 6lbs of pressure. Try and maintain the depth first obtained at the surgeons office.

You had your srs 4 years ago and I dont know the depth you had originally nor the depth on the second surgery. What depth are you looking to achieve?

It is very difficult 4 years after surgery to establish a depth more than an inch extra, and even that will require active dilation. We also recommend that even with constant sex you still need to use the medical dilator 2 to 3 times a week.

The only solution for a depth of 6 ins or more, at this stage,  is by Sygmoid Colon surgery. This as a secondary surgery is very effective and is also self lubricating.

I'd agree with Warlockmaker, colon vaginoplasty is the only option at this point, but its going to be more expensive and there are potential problems with it.

I'd suggest you don't go to Dr Thep again. I've heard he is well regarded among Thai women for breast augmentation, but not SRS - women go to him because he's very low cost.

I believe Oestrogel is not supposed to be used vaginally, but it is a lot safer than using a contraceptive pill for HRT. Some women say to use premarin cream vaginally, but personally that sounds like a bad idea.

With the Colon surgery, they use a portion of your large intestines. While relatively rare, the intestines can heal incorrectly requiring an additional surgery to correct and healing time could be longer. It is a more invasive surgery however dilation isn't as intensive and it's self lubricating. Sometimes it over lubricates and you might need to wear a pad if that is the case. It is a relatively old surgery and I first heard about it in the very early 1980's in my therapy group.

I've heard plenty or reports of bad smell, but I've also heard that it doesn't last. I don't know what to believe, but you'd want to do some research on it. There's not many posts about this type of surgery as its not common. There's some posts on this site about Olmec in India you could look for.

Could you post about having SRS with Dr Thep? There's very little known about him outside Thailand (or maybe that's just on English speaking sites that I read).

A close friend, living in Bangkok, had the normal srs surgery, then 2 years later has Sigmoid colon. This surgery is much safer and easier as a second surgery. She recovered quickly and the odor disappears after a month. She is very satisfied and advises girls on this procedure for us.  Great depth 7ins, self lubricating, she dilates rarely but dilation not necessary if regular sex

This surgery as a secondary surgery is not as uncommon as one would think. Its quite expensive but your original surgeon would give a significant discount, cost as a secondary surgery is usd 7 to 9 thousand.
