I'm 10 months post op MTF. After surgery my surgeon told me I had 6"  I went to have sex with my partner and he bottomed out  I measured the dilator after I finished dilating and I only have 4 inches and it feels like I'm hitting bone. Has anyone heard of this problem before. I had  heard nothing about the pelvic possibly being an issue prior to surgery. I hope they can fix me because 4 inches, I can't live a normal life like that. Thank you for your input.

Welcome to Susan's Place. If you are small in size, it's possible for the surgeon to be limited in room however if the surgeon indicated there was 6", it's not likely. One possible problem is not placing enough pressure on the dilator. It takes about 4 pounds or 2 kilos of pressure to maintain depth and you might be able to push a little harder after you have healed. It is possible to redo the surgery but it is more complex than the original  surgery. For now, you should verify your dilation is correct and if not, attempt to correct it that way.

While I agree that dilation with pressure is needed to keep/develop depth after surgery... I have to wonder if the initial measurements after surgery are not "inflated' due to swelling. In my case I never was able to get the same depth I was told by the nurse based on my first dilation.

Also I am not sure how much vaginal depth depends on body size... I have a large build and the most depth I ever measured, even when I dilated regularly with pressure was between 5.5 and 5.75"...

Now 19+ years later, I dilate once in a blue moon... But when I dilate I can quickly get back to 5 to 5.25" , but ONLY that no matter what...

I have no idea if that is enough for satisfactory intercourse with most guys, as that is not something I have to be concerned about.

With work over time, I suspect she should be able too get at least another half inch... But I don't know how much more... or how much is "needed".

I don't know enough anatomy to know how much more possible  "room" she has... I suspect a good doctor might be able to figure that out... though finding one who would have a clue might not be easy.

It sounds like you've been dilating incorrectly. You're supposed to dilate to the depth the surgeon gives you every time you do it, and you shouldn't be suddenly noticing that you're 4" rather than 6".

I don't really know what to do about it, but at a guess dilate every day, pay extra attention to the depth you get each time, and aggressively try to increase it over time.

At 7 months post-op I'm at 5" of depth, I started with 6½ which obviously I was very happy about and I think the loss of depth came from being fairly gentle with the dilators, that last half inch tended to feel sharp and I was quite worried about overdoing it and risking granulation tissue.

I'd been down to something like 4.5" and realizing that's still ok but not what I want, for the last month I've been pressing depth pretty hard and it seems to be paying off. This makes sense, the neovagina is created by what surgeons call blunt dissection -- basically using fingers or a problem to separate tissue.

So I'm hopeful to be back to 6" I'm deep enough as it stands to take almost any penis I've ever seen, however I can't quite hold in 2 been waiting balls and that's something I want, also I'm not quite deep enough to fully insert most toys (which are mostly larger than the average guy).

When my GF bottoms out during strap on sex (doesn't happen in all positions), it feels fine, definitely no sensation of hitting anything hard and if it's sometimes on the edge of pain, I like that ok in sex.

At 1 year I was told to always use the #1 dilator for 5 minutes to maintain depth then go to #4 for 25 minutes. I push about 4 pounds. I was 3.75 inches post op and could not push for 2 weeks when dilating then I could push a pound for about a month then push as I liked within reason. I am at the 5th dot of #4 dilator inside my canal when I dilate. I think that is  5 inches.
