Hi ladies,

Hope you all are fine and happy. I am on HRT. And i have been taking my dosage 1 time a day like all the tablets combined.  As per guidelines of this forum i cant write the exact dosage but i will give an example to deliver my question clearly.
Let suppose you are prescribed Spiro and Estradiol at doubled amount can we take the doubled dosage at the same time or we should divide it by 2 times. Like 1 dose in the morning and other dose in the evening. Just concerned about the health. I hope i made it clear for anyone to answer.

Hugs and love,
Cedie

Maybe doctor instructed me that take one dose in the morning and one in the evening for E and Spiro. Might be worth asking your doctor. Especially E breaks down pretty quick, so taking a dose every 12 hours helps keeping levels stable. My doctor prescribed dosages so I can take them twice daily.
Only exception is Progesterone which I take in a once daily dose...

I just had a doctor's visit on Friday and he increased my dose for the 2nd time and I asked if I should split it up and he said just to take it all at once, but I've seen others on here say they split it up. Honestly, I think you're fine either way

I take my estradiol sublingually and take it three times daily, I get hot flashes later in the day if I do it all at once in the morning.  I also heard once that levels too high aren't anymore useful then steady more moderate levels.  So, if that's true, splitting the dose would be better.

You should definitely split it up.

The blocker has a longer half-life and that you probably could do only once a day together, but IMO every 12 hours is best.

The E though has a much shorter half-life and your levels will have high spikes and long lows after the first 6 hours or so. You should definitely split it.

I was taking my e 3x a day before they reduced my dose to 2x a day and I noticed a big drop in my mood and stability when I didn't have that middle dose so I asked to switch to injections instead.

Some have had fine results taking it all at once but I can speak from experience when I did that for 6 months last year my levels were terrible the next time I had a blood test and they immediately told me to split all my doses.

How has it been with injections? I'm about 3 months next week and thinking about switching. I'm doing sublingual 3x a day right now, 8 hours apart.

I have a hard time giving myself the injection tbh, but I am learning a lot too so I like it. My results were pretty good for the 10 months I was on sublinguals but I am much happier not having to worry about the pill timing or waste of swallowing too much now. Beyond that it is a little too early to tell what is affected by what. I am only approaching the 2nd month on injections but my hot flashes went away along with my exhaustion and the mood swings I was experiencing.

How often to take any given medication depends on two things. The first is the elimination half-life of that particular drug. Short half-lives mean taking the drug more frequently. The second is does that particular drug give better results with a steady blood level or are cyclical peaks and valleys preferred.

In the case of Estradiol, oral and sublingual have relatively short half lives, so we take them several times a day. Injectable Estradiol is usually given every week or two because of the longer half-life due to the slow release of Estradiol form the oil based mixture. Patches and gels are another story, but the principle is the same. Short half life, more frequently. Long half-life less frequently.

Spironolactone has a half life of about 1 or 2 hours, but has active metabolites that last much longer. So daily dosing of Spironolactone is acceptable unless you have uncomfortable side effects right after taking Spironolactone. Then you would cut the dose in half and take it twice daily and hopefully that will reduce the side effects.

The bottom line is answered by one simple question. Are you seeing the desired results?

For HRT that feminizes, exact doses are not important. You just need to be in the normal female range for several years and just let the magic happen. There is no need to try to push or tweek your HRT.
