I had an encounter with a new Hygienist at the dentist today.  She gave me the impression that she made me as a transgender woman.  I still present as a male in public, except for special occasions at the moment.  Her conversation revolved around being PC with customers and that she has to be careful not to offend anyone.  I didn't bring it up, she did.
I've only been on E for 7ish months.  Is it possible that someone can just know by looking at you?  Or did having her fingers in my mouth help?

Perhaps it could be the effects of HRT softening your skin, or perhaps the fat redistribution.

I know that after just 2 months of HRT, and presenting as female, someone asked if I was transitioning to male. Go figure.

After just four months on hrt its pretty obvious. I could wear mens clothes, but quite simply people will see me as... odd. Therefore I don't bother and let people see me as trans. That way at least its not confused questioning.

With a few exceptions, those who transition from M to F later in life (older than 35 yrs) will be recognized as trans.  HRT alone is not enough to undo decades of testosterone effects.  So yes, your hygienist knew you were trans, but if she's nice about it and still treats you like a woman, does it matter?

She wasn't presenting female.

For me lots of people noticed that something was up, especially after I stopped caring about keeping my voice low. Nobody said anything though.

I don't know know where you live but in the UK the girls from the North have deeper voices than a lot of the men

I'm another dirty yank and out here voice pitches are all over the place. I figured I'd nail that to avoid any other shortcomings from getting me clocked since I'm insecure like that. Though even on days where my pitch isn't perfect inflection makes up for it.

She probably felt like giving extra attention to customer service. That's a good sign to show respect especially if she's younger. Not that age matters at all because it doesn't and shouldn't but it's just a sign of respect.

I'm probably way off/wrong though

I remember once when I was first transitioning and had just started on HRT I was taking a load of scrap steel over to the scrap yard. I had spoken with the woman at the weigh-in gate many times and we were on a first name basis at that time.  I was wearing a woman's tee shirt at the time.  Out of the blue she asked me if I had a different name I wanted to be called. BUSTED.  For some reason she knew right away what was going on.  At the time I remember I was pretty tickled by it.  Nowadays we chat like girlfriends whenever I go there.

When I started hrt my dad didn't tell my grandpa about it. I was as androgynous as I always was but I didn't start presenting as fully female for almost a year after starting. I was about 3 months into hrt and I didn't look any more feminine than I usually did but my grandpa somehow knew. He told my dad he thought I was taking female hormones. My dad told him he knew I was because he had taken me to the Dr to start them. My grandpa went nuts and it ended in a huge fight between he and my dad. My dad had the attitude that I was his child and if I needed to transition to be happy he didn't give a ### if anyone else didn't agree with it or what they thought.

Honestly I think she was fishing.  She put out statements and comments that I think she designed to provoke a reaction.  She mentioned her child thought she was racist, which invoked a questioning look from me.  She stopped with that tact immediately.
