The way I read it is: it's the actual patch used for non-scrotal skin that allows for increased permeation of its product into the skin and hence, increased absorption. Read the conclusion, it explains it all pretty clearly.

I had absolutely no incentive or inclination for researching this topic as much as I did as it means little to me and I only fell into this pit because of the absurdity of a person placing hormone patches on their penis, and yet after 800 years of faffing about I finally learned how to gain access to full research articles by using sci-hub. Which makes this all worth it.
