Hi all, I wasn't sure where to put this but here goes:

An amab, nonbinary friend of mine is considering feminizing HRT for the long term. It has all the effects that they want and need, but they do not want breasts. We are wondering if there is a type of preemptive top surgery for them to get so that the breast-developing tissue/glands are removed before they are affected by the estrogen. As an alternative, they are considering (although iffy about) getting top surgery after going on E. If the former is possible, which tissue would be removed, and what is the surgery called? If the latter is the only possible option, how developed must the breasts be before getting top surgery?

     I am so very glad that you have become a member here and that you have started posting in the Forums.

Gynecomastia is the development of breast tissue in males due to elevated levels of estrogen.  The developing breast tissue and fat can be removed surgically.  If you're interested in what is involved in this surgery, this YouTube video should be helpful.

HRT is NOT a guarantee of breast growth. Take a look at the HRT subforum. There's no sense planning for problematic breasts that haven't developed yet. Your friend should talk to a doctor about their goals and expectations. See you around the site!

Another alternative which may offer what your friend is looking for is to only do part of the HRT.  Skip the estrogen supplement.  Just do the spiro which lowers the T and gives the estrogen naturally produced by their body time to make smaller changes.

A note of caution is needed here.  I did this and didn't like it. The body needs a sex hormone.  Either E or T.  3 months being on Spiro then I  started low dose  E. Much better.

Very true. No estrogen will leave you feeling flat.

From what I have read, your bones need either T or E to stay healthy.  I wondered about doing Spiro without E and was told removing the T without adding E might cause me bone density problems down the road.  I too like the effects of lowering my T and adding the E but am not really ready to explain a pair of breasts to my friends and family.  I do understand your goal, I am just not sure it is a healthy path.  As mentioned, some never even get breast growth, or so little that it can be explained away with mild gynecomastia.   I pass by an older guy in the YMCA all the time that must be sporting C cups and no one gives him a second look.

I agree that the best course of action may be HRT of both E and T Blocker such that you gain some E aswell as reducing your T. Almost all of us start on low dose HRT and my advice would be to simply do the same and see to what degree you get breast growth and take it from there.

There are many Susans Members permanently on low dose HRT and I hope your doctor would understand your intentions and maintain that low dose.
