AUSTIN, Texas — Health insurers that cover gender-affirming care would also be required to cover any adverse effects or reversal procedures under a bill the Texas Senate passed Thursday.

"If the health insurance company pays for that transition, they have to pay for the damage caused by that, for the complications and for the detransition, if that's what the patient wants," Sen. Bryan Hughes, a Mineola Republican who authored the proposal, said shortly before Thursday's vote.

Senate Bill 1257 — which passed 20-11 — would require health insurers that cover or previously covered gender-affirming care to provide coverage for all adverse effects, necessary annual testing or screening and care needed to manage, reverse or recover from gender transition surgery or treatment.

It would require such coverage regardless of whether the person who received gender-affirming care did so while enrolled in the plan at the time.

In a bill analysis, Hughes said his intent is to help patients who either have complications or regrets from transitions by requiring insurers to "provide coverage for necessary medical interventions."

"Many of these individuals require extensive medical care to manage or reverse the effects of previous treatments, yet they are frequently denied insurance coverage, leading to insurmountable out-of-pocket expenses," he wrote. "Without this coverage, patients face significant health risks, including hormone imbalances, surgical complications, and psychological distress."

A survey of more than 92,000 transgender people in the United States found that the vast majority reported more life satisfaction after transitioning, despite facing discrimination in healthcare and the workplace settings. ... People who had surgery to match their gender identity were even more likely to be satisfied with their lives, the survey found, with 88% of people who had at least one form of surgery to match their gender identity reporting that they were "a lot more satisfied" with their lives. Another 9% of respondents were "a little more satisfied." One percent of people said that surgery had no impact, while less than 2% of people said they were a little or a lot less satisfied after undergoing gender-affirming surgery.
