Keyhole, they basically leave the nipples intact and remove the breast tissue from around the areola. Scarring is minimal, but most need a lot of revisions.
Double incision (which is what you were trying to think of, Nero), is two large cuts, remove all bothersome tissue, and the nipples are removed, resized, and replaced. The chest is sculpted to look male. The scarring can be more significant, but you don't usually need revisions unless you're quite overweight. I had DI and, although you can see my scars, they aren't really that bothersome and not particularly noticeable except up close, especially if I'm tanned. Avatar picture is me a year and a half after surgery, with a tan.
And I don't know about keyhole, but DI was not a particularly difficult procedure. I was sneaking out for a smoke 45 min after waking up from anaesthetic (yeah yeah I know, bad idea and all). The next day I walked up Lombard Street Hill and around the piers in San Fran for a couple of hours.
