How common is it to have bottom surgery first or as the only surgery? I'm sure it's not very common, but has anyone heard of anyone doing this?

Would it be harder to do it in this order? (I guess I mean... would surgeons have a problem doing bottom surgery if you haven't had top surgery, even if you have a smaller chest and might not get top surgery?)

Depends on the surgeon, I'd imagine.  But to me if you have a letter, you have a letter.  I know of no one who has done bottom first/only, but that's not to say it can't/hasn't been done.

I'd imagine it is common with people who's breasts were so small they broke down enough after taking T? I don't know. I've never heard of an FtM having bottom before top.

I'm considering medioplasty before top surgery because I've continually asked doctors if freeing the clitoris would give it more room to grow larger. I'm not sure how the development works, but if freeing if gave it an ability to grow larger, I'd seriously want to have medioplasty before top surgery.

I do think that approving the surgeries in that order could be problematic. But speak with your therapist, explain why you want the process done this way.

Letters for surgery generally are not procedure specific.  A surgery letter is a surgery letter- they cover top surgery, bottom surgery, lipo, etc etc etc.
