Well, O.K., I don't have it in hand, but he's sending it to me in the next few days.  Tomorrow I need to see if I can get an appointment with the Endo.  Actually, I have an appointment with a local favorite Doc, but she's booked 3 months out, so it isn't until the end of May.  :(  But my therapist gave me the number of another doctor who used to work closely with the one I'm booked with, so I will try him first thing tomorrow and see if I can get in sooner. *fingers crossed*
