Nope, lots of colleges have dealt with people transitioning in school. I'd avoid a women's college, though. That could be awkward.

Even if you have your changed first, you still have you other name on your transcripts. So I would say someone at the college will know.

Depending on how far along you are in transition, there could be some housing issues. If you started transition the summer before college and after high school, you'd probably be best to apply with the name that's on your high school transcripts, then tell the college after you're accepted that you go by <male name>, or have legally changed your name to <male name>. Then after your first year, you'll be as anonymous as you want to be. Just some profs would know in first year because of the name change.

If you started transition during college, same thing, just tell the admin when your name changes.

With housing, if you are trans, most colleges will give you a single room in coed housing so there's no roommate issues. Or if you live off campus there's no issue at all.

I have some specific experience with this, as I am in college right now and transitioned before I went. You're in the same position I was in a year or two ago. My advice is to apply as a female, then tell the college you're transgendered and ask them to change your records. My college did just that, and now all my records--and my student ID, which serves as a useful photo ID so I don't have to show people my female ID--say Andrew.

You definitely should apply to a GLBT-friendly school--most schools on the East Coast are very open and will accept you as you are. You needn't be "out" to everyone--I'm not--but you'll inevitably have to tell some school administrators, and definitely the health center. Your SAT scores, health records, and transcript will all be in your old name, but most people never see those, so you should be nice and stealthy.

I wish you good luck on your transition! I hope it doesn't interfere too much with your academic life--keep those grades up, buddy.
