A few years ago, my then-50-something father-in-law went to Thailand for gender reassignment surgery. 

(You may be interested to learn that Thailand is a very popular choice for Westerners seeking gender reassignment surgery.  It's a lot cheaper than having it done in North America, and there are no pre-op requirements - in Canada, in order to qualify for gender reassignment surgery, you must undergo a lot of psychotherapy sessions and live full-time as your chosen sex for 2+ years before you qualify for the operation.)

The operation was successful, and now my father-in-law lives full-time as a woman.
